The application asks if I have recurrent back pain. I don't know how to answer. Sometimes my sciatic nerve hurts like crazy. Is that recurrent?

Also, it asks if I have Mass, cyst(s), or lump(s) in any body part. I have these little bumps all over my hands, does that count?

If you are applying for insurance "nothing"is wrong with you otherwise you can be denied. If you already have it, claim your ailments since you are already covered. You will be taken care of.

Only reply "yes" to a question that you have previously sought treatment for. Otherwise you will be either rejected for undo risk, or you will have that diagnosis excluded from 2 years of treatment as a "pre-existing condition".

then answer "yes" and explain what was done and how it was treated. If you lie, you can go to jail or be fined for insurance fraud (yes they can really do that!). If you show them up front it was a minor condition, then there will be no big deal. 50% of Americans have some "severe" LBP each year

In all seriousness, say "no" to the recurring issues and then go get it looked at. Seems dishonest, but they'll probably reject you if you say yes.
