If the chain skips under load and it does not have stiff/twisted links. It's worn out.

Chain links are supposed to be exactly 1/2" long each, so the best way to check if a chain is getting worn is to hold a ruler up to it and measure the length from one pin to the corresponding part of a pin that's 12" away. It should measure exactly 12" when the chain is new and if it's more than 1/16" over that then you should consider replacing it. Waiting until the wear is worse and makes the length over 1/8" longer is likely to result in increased wear of the cogs in the cassette and require a replacement of that as well.
Skipping refers to the chain not remaining seated properly on the cog and jumping ahead by one link from time to time as you pedal. This is most likely to happen when you wait too long to replace the chain and therefore the cogs become worn. Then a new chain doesn't properly engage with the teeth on the worn cogs and it starts skipping - so you need to replace the cassette as well.

A chain will skip or "auto-shift" when it shouldn't due to cable stretch. Usually adding tension to the shift cable with correct this.
If the rear derailleur is properly adjusted and the skipping continues this is usually a sign that the chain is not meshing with the cogs due to excessive wear. A new chain on a worn out cog will skip even worse.
