The mid-range Campy BB cartridges have flanges on both cups. Is one flange just for tightening, and is not seated against the BB shell when the cartridge is gripped tightly? If so, which one is preferred to sit out a bit?

For the mid-line bb's with the drive side cup built into the cartridge (Centaur and below), always seat the side with the cartridge first and fully and torque it to spec. Then install the nds cup and torque it to spec. If the bb shell is the correct width, both cups should be flush. If it's too narrow, let the nds cup stick out a little.

For the Record and Chorus bb's with two separate cups, install the drive side cup and torque it to spec. The slide the cartridge i andinstall the nds cup. Again, if the bb shell is correct, both cups will be flush.

Makes sense. I didn't realize that the ds cup was part of the cartridge.

If the BB shell is too wide, will the unit tolerate the nds cup seating against the shell and not the cartridge, or should the shell be faced enough that the cup seats against the cartridge?

Too wide shells are a rarity and I wouldn't worry about this eventuality. If anything, shells that have been faced too agressively and are too narrow are more likely.
