what exactly is and does compact geometry do for a rider, and what is the difference between an italian setup and us setup if any?

Compact Geometry frames have a sloping top tube whereas traditional frames have a more horizontal top tube.
Compact geometry allows some riders to be fit to a bike easier than on a traditional frame. (i.e. Long arms and short legs.) Standover height is less an issue with a compact frame.
It also allows manufacturers to reduce the number of frames they have to build (48,50,52,54,56,58,60,62 vs S,M,L), and LBS,s don't have to stock as many to get someone fitted fairly decent.
