Hi, I just got a surly LHT and routed my gear cables on that black plastic thing under the BB shell.

Does this thing need to have the cables run through little plastic sleeves or anything? Or is it just fine to run them as is. I am using downtube shifters (no housing except the loop on the RD end).

You don't want to do that. No place on a bike sees more grit and dirt than the underside of the bb shell and any grease there will be a magnet for all kinds of abrasive crud. Use a thin film of very light oil or, even better, a dry lube.
