Brakes came with two serrated washers per caliper. With two washers on mounting bolt, the brake looks like it's sticking out too far - do both washers really go on?

They come with more than needed in case you need them to clear, say the fork. Use as many as you think is necessary. If there isn't enough the brake pads/shoes may hit parts of the frame even when it's properly adjusted.
