Is it the shape of the bar? Is it the orientation of the riser? Might it depend on the overall design of the bike frame?

I think a "tweener" bar is generally understood to be an above-seat handlebar where your thighs are "between" the two ends of the handlebar. Think of Bacchetta's setup. That's the origin of the name.

That's what I understand when I hear it and what I mean when I say it, but I don't really understand why it's called that as in normal parlance the word "tweener" usually indicates a compromise between sizes.
It's also known as a "superman" bar because it's meant to be held at the bar ends with your arms and fingers fully extended (like superman), but I like it because you can also get a variety of grips and postures by grabbing the bar just about anywhere else.
