i'm shopping for a new lid and think i'm going to buy a giro pneumo.

the issue is that both large and medium fit me, but the large required the rear sizing thing to be cranked down towards the smaller end, and the size medium requires it to be cranked up towards the higher end.

in winter, i wear a lycra skull cap under my helmet, but it's very low profile, and if anything, holds my big head of hair down even tighter thatn it naturally would be, making my head smaller!

which would you prefer?

I'm in the same boat and I use a Pneumo. I went with the large because sometimes when the weather is cool but not cold enough for a skull cap I'll wear a hat. I also will wear a sweat band so the larger helmet offers more adjustment.

Same thing over only I went with the medium size
I liked the smaller less weight factor for me

try it on with the cap first.

Check this. Both may feel fine as far as loosness / tightness goes. But, which one sits on your head better from a protection standpoint? With the helmet on and adjusted how many fingers is it from your eyebrow to the helmet? More than two is too much.

That may help you decide.
