Is there a right-hand crank on available that enables a chain guard to fit tightly
near the crank pivot?

The stock crank does not allow for a chainguard to fit between the chainring and the crank.

Why don't modern bike makers, with few exceptions, integrate the chain guard
when they use an internal hub? It seems obvious???

The only one that comes to mind for new ones is the velo orange grand cru (http://store.velo-orange.com/index.php/grand-cru-110-fluted-double-crankset-34x48t.html), they look more old-school. The other option is finding a more vintage crank.

If you haven't bought the new bike yet, I think I've seen some IGH bikes that comes with full chaincases now. I am builing up a 5 speed hub bike for the winter and plan on fabricating a chaincase from scratch, but I will actually be following this thread too.

Have you found any chaincases for sale? I think yellowjersey has the old, heavy roadster style for sale.

The Breezer Uptown bikes in some models come with full chaincases. I have seen web listings for plastic full chaincases that are made in Holland or Germany too but do not know of sources for them. You might check with Clever Cycles in Portland as they are european cargo and utility bike specialists and might be importing something or have sources to obtain chaincases or complete bikes with them.

If you choose the right IGH bike then the Hebie Chainglider is a solution though the available front chainring sizes supported are limited. It should fit most cranksets though you might need to remove the anti chain drop post that sits between the chainring and crankarm on some cranksets.

Not knowing the bike and/or crank you'll be getting, I can't tell you how well this will work or what modifications may be needed to make it work.
