The domed fork and stay ends indicate not Italian, but with what I see it could be made in UK or North America. What is the width and threading of the bottom bracket shell?

It has shimano 600 cranks. Looks like european bottom bracket

This bike has shimano 600 grupo. Cinelli stem and bars ans it had a cinelli chain stays decals. I dont have any idea but both frame and fork drop outs are campagnolo

Lots of US, UK, Canadian, and Japanese builders used Campagnolo dropouts and Cinelli lugs and fork crowns. How wide is the bottom bracket shell--68mm or 70mm?

well i check and is 2 5/8" inches i guess multiply by 2.54= 68mm

Not Italian. You can confirm this by removing the cranks and bottom bracket. The right hand cup on British-threaded (non-Italian) bikes is reverse-threaded.
