So I am working on a winter project bike. It has the old Japanese/American standard quill diameter of 21 mm. The handlerbars that I want to use are 26 mm.

Has anyone ever seen a quill stem like this? Is this something that I will have to machine myself (I have the facilities)? I am thinking that if machining is required for this combination, It would be better to machine the clamp ID, rather than the quill OD...

The 21.1 quill was due to the fork steerer tube thickness and and the necking of the headset top (lock) nut. Do you think it might be easier to find a different fork that would allow a 22.2 quill with 26.0 throat? The headset (except for the locknut) should be identical.

So, if it is just the ID of the lock nut and steerer tube, I wouldn't need new races? And the OD of the 1" forks should be the same right?

Exactly. and it is only a lip on the locknut.
