Yes, if it's causing the cable to not move smoothly.

It could, but it would depend on the nature of the crack.
I've seen some plastic guides whose curve didn't match the curve of the bb shell. You can get it to fit by screwing down the mounting screw hard so the plastic will bend to conform to the shell. But if it cracks or the screw strips, you have a plastic guide that is acting like a leaf spring, especially in the small ring or small cogs where the cable tension is at its lowest.
