My FD worked fine with my old friction shifter on the DT.
But now, trying to connect the Ultegra handle I notice that I can't pull the cage against the spring, since the cable comes up from below.
I realize the derailer is a "top swing", but how on earth did it do that without a problem before?
The cable ran in the same guides as it does now!
I can't see any way to have it pull from above.

It's been a while since I worked on a top swing, but you posted some nice pics! Looks like you need a longer cable run (i.e. new derailleur cable). And then take the tag end of the cable and thread it from under and around the outside of that black pulley with groove on it, under the little cable retainer tab on the clip (keep the cable in the pulley groove) and then bring the cable down to the clamp from a few cm above. Before tightening, make sure the shifter is in the most relaxed position and not picking up any cable. Snug the cable to the FD, and without shifting, just laterally pull w/ your hand on the cable. Does the FD pivot? If so, the shifter should now be able to move the FD. You may need to do some limit adjustments and the like. But you should be most of the way there.
That FD is made to work as top or bottom pull, and for bottom pull you need to run the cable in that grove around and down to the clamp.
