Do they measure from the top, middle, or bottom or the BB? Then where to?

I think the 80s ones were from center of the BB to top of the seat tube, including the extension.

My '91 3.0 frame is a "56cm" and measures 56 from the center of the BB to the top of the seat post. Center to center is approx 54cm

I have a 1987 R500(63cm). It measures 66cm from center of BB to top of seat tube.

From top of BB to top of seat tube is 63cm... if that means anything

The serial number contains the size of the frame as well so if you have a frame already see what the size is supposed to be and then see which measurement corresponds with that.

Drummerboy1975, My '89 crit bike is a 56 cm frame and measures 56 cm from the center of the BB spindle to the top of the top tube.

PS My bike is pretty much spot on using the 1990 Cannondale catalog.
