That's something I generally buy locally, as sizing can vary and I need to be able to try them on. Not sure about wide widths, but I think you are someone who definitely needs to be able to try them on first. I'm perfectly average at a US 9 1/2, Euro 43, and right on the border line between regular and wide. I have an easy time buying shoes.

I buy mine from a regular shoe shop. But then I like to be able to jump on the bike and go, so I use plain pedals.
Buy where you can try them on, if a LBS carries your size.
Most bike shoes stop at size 48 (12, American). I've mail ordered up to three pairs and sent the two pairs back that didn't fit.

I have a wide foot, and Shimano R087W shoes in size 48 were just about perfect. Size 48 supposedly translates to 12.5 American, and I usually can get by with a size 12.

Got mine from Performance Bike. They fit great and didn't break the bank!
I too have wide feet (11 EEE) and can only wear Sidi mega width or Lake extra wide.
I usually look for deals online.

Mine are 11 1/2 EEE. Those of us with snow-shoe feet might be well advised to avoid mail order but I've had success with Vittoria Ace and Diadora Aerospeed, both size 46. These run a little large and have a wider toe box, the Vittorias, more so than the Diadoras. This only applies to these models within the brands.
I also force pseudo lasts that I made from pine, which are the shape of my feet, into the shoes and leave them there for at least a week, before using them. This makes the break-in period very short.
