A bike shop can order one if they don't have it in stock. FSA makes one that's available through QBP, for example.

Profile Designs sells the "Gap Cap" that fits both 1" and 1-1/8" steerers: http://www.profile-design.com/profile-design/products/accessories/all-accessories/gap-cap-.html

I had an early version of it installed in the 1" Cr-Mo steerer of a Kestral EMS Pro fork until I determined exactly how short i wanted to cut it. It worked well.
