As a kid i had a piece of wood stuck through the side of my foot. About the size of a pencil. They put me on the kitchen table and held me down. Dad took it out.
Yep the teenager is hurting for sure. Hurts like a dog bite i suspect or worse.

I had a fish hook go through my finger when I was 7 or 8. Fortunately, my Dad being an Electrician, we had a pair of "Sidecutter" or "Linesmans" Pliers in the Kitchen drawer, so we were able to cut the barbed end of the hook off before pulling it out.

Lucky you. My mom just yanked the fishhook, barb and all, out of my arm.

A brake lever seems that it would be much more painful than a fishhook though. At least she had pain medicine.

Modern first aid methods say that if a person is impaled or stabbed with something, it should be left in place, an ambulance should be called, and any removal of the object should be left up to the professionals.
