I want to say it's SoundJuicer in Linux these days using an ffmpeg or lame backend. MP3 because the format is pretty much universal, and I use VBR encoding with an average of 160 or 192, I think.
But it's been several months since we've had an actual CD come through here. The last few albums have been downloaded via Amazon.

I use CDeX. I usually set it to the highest quality variable bit rate. Always MP3s.
I rip to FLAC and then sometimes to MP3/320 CBR. I have a modded 5th gen ipod w/240 gigs of space and Rockbox firmware (plays FLAC). Works for me.

I use MP3, 256. Sounds the same to my tin ears as the higher bit rate, or the original CD, unless I'm in a really quiet place. In the car, no difference.
