I get my chain slipping off and immediately back on to one of the middle back gears when I lay on the pedals and sometimes just cruising on even road. It looks like the chain lines up fine with all the gears as I shift up and down with the bike on the trainer, and I get no chain noise as if it's scraping between gears, but for some reason it slips on the road. Where does the problem sound like it's coming from? Bad adjustment, chain? etc...

How are your derailleur cables? Remove the cable stops from the frame, this will free the housings up and let you slide them down the cables. Run a little oil or very light grease onto the part of the cable that is inside of the housing during use, also check for rust, dirt, kinks, or broken strands. If in doubt, replace the cables.
A dirty, sticky pair of cables will do more than almost anything else to cause shifting problems.
