Yep, 1960's/70's style Stingray or Krate frame, fork is an aftermarket bent springer. Considering how popular and expensive (in 1960's/70's dollars, it took a lot of news paper deliveries and lawn mowing and snow shoveling to pay for one) the Krates were, (thus fairly rare to be seen on the street or in a school's bike rack), it is probably a Stingray frame. The Krates had a 16 inch front wheel, rather than the standard 20 inch of the Stingrays.
You can get all the parts - including the frame and fork - from pretty much any bicycle 'Low Rider' shop/store online.
