A stem with no room for top cap?

It's impossible to say, but as is there's no way to adjust the headset except by tightening it firm and tapping it down. That doesn't make sense, so I suspect that you're supposed to remove the extension, or set it at 90° to expose the top, fit the riser which has some kind of top cap or draw system inside, adjust the headset and clamp the stem, then assemble and/or the upper end.
Of course I don't know that for a fact, or I would have said so, but decent design demands some kind of micro adjust provision. Then again there's lots of ill conceived stuff out there.
Why don't you ask the seller or maker?

In one of the pics, there seems to be a bolt which extends down in the middle of it, just like would happen with a regular stem. I bet there's just some cover or cap over it in the rest of the pics which show the top.

The first couple of photos show some sort of rubbery-looking flap, or similar. That flap (or stopper?) probably gives access to the bolt.

Well thank you for your responds. I bought the stem. It will arrive tomorrow. I hope it fits. I spoke to the companyand they said it doesn't need a top cap.
