Maybe some photos and a picture of the headset. 1" Threaded? 1 1/8" threadless? Integrated headset? Loose bearings (caged) or sealed bearings?
Did you have to use a bearing press or tools to install the crown race & headtube cups, or did they slip in by hand?
Oh, if it is threadless, sealed, then was there a cone shaped piece on the fork crown as well as on top of the upper bearing, below the stem or spacers?

In my experience, when a headset cannot be adjusted without either slop or binding, it means the head tube faces and/or crown race seat must be milled so they are parallel and co-axial. This is a job for a bike shop.

or the cups got warped during installation, or were not seated up correctly to the head tube.

What frame is it, what headset is it ? Did the headset come with the frame ?
Things to check -
Either of the bearings cartridges have not been inserted upside down.
The centre compression ring is not upside down.
When the headset is tightened, does the top cap peice come into contact with the head tube ?
Have you fitted the crown race ring to the fork ? is it fully on with no gap ?

one other thing to check... is there a plastic seal ring on the bottom race? VERY EASY to get that seal off center when slipping the fork into the frame.... and the symptoms you list sound exactly like when a seal ring moves/gets pinched....

I got a Tange headset with that plastic seal ring. If you tighten it properly the seal ring gets pinched and creates friction between the stationary cup & moving cup surface. I don't like a loosey goosey handlebars so, I just leave it like that.

Sounds like you installed the headset wrong.
Are the bearings the right way around?
