Loose Headset

I have a threaded headset that is always comming loose. I have repacked with greese numerous times and have tightened it correctly with two headset wrenches. After about 2-3 rides it comes loose. Is this a sign of a worn headset? is there something wrong with the fork?

Make sure the threads on the cone & lock-nut are clean & dry, then use a little blue loctite on 'em. Let it harden a couple of hours before riding it again.
