Chain Skipping

When ridng hard, in 54 x 15 or lower, my chain starts to skip. Last time out it skipped twice then fell off. I put it back on and the same thing happened. Any thoughts?

How old is the chain? And the cog? How many miles for each? Where does it fall off? The back or front?

Chain and cog are both 2 years old. 2500 miles. Falls off the front.

Is it possible the cog could be bent slightly?

Had similar problem. Turned out the rear derailer needed a good cleaning. It had accumulated enough crud to keep it from flexing correctly to keep tension on the chain.
