Broken Quick Release Skewer
I was putting my front wheel on to go for a ride, and as I was closing the lever, the shaft separated from the cam and the skewer completely fell apart.
Has anyone else seen this type of failure? What is the cause?

On some skewers, the lever is held in place by a c-clip on the lower end of the lever shaft. If this clip falls off the entire skewer comes apart.

There is no clip on this one. The shaft screws into a threaded hole in the cam. The shaft broke right at the end of this hole, leaving part of the shaft stuck in the threaded hole.

If it was a Shimano skewer, Shimano ran a recall last year on some of them due to breakage. Google "Shimano skewer recall" and you should be able to find a link.

I avoid boutique Ti or Al external cam skewers and stick with either Shimano or Campy. So far they have been completely reliable. I did have the c-clip come off of a Shimano skewer a few years ago but that didn't cause a problem while riding and I discovered it only when I removed the wheel.

The skewers were the ones that came with my bike, Cannondale SR500, fitted to Cannondale Earth hubs.
Top part of picture is the intact rear skewer.
I replaced them with enclosed cam skewers from Quality Bicycle Products (Dimension).
