Adding a Granny Gear to Ultegra Crank

Are the only things that need to be changed to add a granny gear to an Ultegra double crank, are the crank and the brifter, or are there made certain chainrings that can add this granny gear without changing the crank, or would the derailurer need to be changed as well?

I assume you mean a third chainring. No, you cannot "add" one to a double, but a crankset with three chainrings is called a triple. You can replace a double crankset with a triple. What ramifications that has depnds entirely on what Ultegra models you have. How many cogs on the derailleur?

A triplizer ring may work. It's basically a special inner ring that contains bolt holes for a granny. You'd need a new BB and probably a new FD and maybe a new front shifter depending on what you have now.

The Truvativ Elita double on my recumbent (20" wheels) has a third ring (not exactly a granny at 39T) added inboard by using longer chainring fixing bolts and spacers. 39/50/63. (38 is as low as you can go with 130BCD)
