right crank arm

Hi hope all are good... Put in a new Token BB and now seems right crank does not stay tight... BB on right side seems to stay tight. It is an Italian BB but have locked it down very tight... and seems still tight...
but after riding bike only a few miles or less right crank arm is lose... it is a Sugino XD crank doesn't have 5 miles on it... have tried two different crank bolts... any thoughts?

thanks a ton ...

Square-taper?

What is coming loose? the arm on the axle, or the BB in the frame?

Is the crankset new or used?

You didn't answer the question.

Either a) you (or someone) didn't torque the bolt enough when it was installed, or b) the arm socket is ruined from being loose.

The "or" in that sentence is an inclusive or. Both a) and b) may be true.

Could also be that you are running a JIS taper crank on an ISO bottom bracket (you did say the bb was italian) depending on the brand and intended use, it may be using the ISO taper. If this is the case, the bottom bracket bolt can bottom out on the spindle before the crank is seated on the taper. Or it could be a combination of that and a worn crank.
