Cable Barrel Adjuster
While stretching and then anchoring a new derailleur cable should the barrel adjuster be set all the way in, all out, or somewhere in the middle ? or doesn't it matter where it's set at ?

For initial set ups I think that it's best to screw barrel adjusters it all of the way in then back it out 1 turn.
That'll give you a little flexability if you need to loosen the cable tension a small amount during the initial set up but keep most of the barrel adjuster's range for taking out cable stretch later. Cables never shrink during use, they only get longer so make sure that you have adjustment range to take care of that.
