Pls confirm specs for formula th21 36h low flange fixed/free hub

Hi, I'd like to use Damon Rinard's spoke calc xls and want to be sure I'm translating the data on formula brand hubs (model th21, 36h, fixed/free) properly.

th21 flange width = 54mm

** is this the same as Rinard's "W", which is "hub width from center of wheel to center of flange"

th21 P.C.D. = 45mm

** is this the same as Rinard's "d" , which is "diameter of the circle through the centers of the spoke holes"

can th21 spoke hole width be safely assumed to be 2.0mm? formula website lists 14 gauge
