Why would a headset go 'click' under heavy braking?

It's a 1" Cane Creek threadless headest, which is about 4 or 5 years old

I'm quite sure it's the headset making the click, because the same thing has happened with the same headset on 2 different frames, 3 different forks, and a few different stems and bars.

It sometimes clicks just once, under heavy braking or going over sharp bumps, like speed humps.
I'm positive everything is adjusted properly
The cups are VERY tight in the frame.

    Thank you

You've had the same headset in multiple frames? I'd start with a new one personally.

It's either loose, or worn. Time for a we headset before it eats a frame.
