Rack bolt, self-extracting.
My alu framed dahon has rack mounts which are over 1cm deep. Ive had rack bolts work themselves lose and fall out. At first I thought someone had stolen one but the replacements came lose as well.
The luggage rack is never heavily loadeed, the rack bolts are greased and tightened.
What is happening and how can I keep them in place?

or a longer bolt and a nylon-insert nut on the other end for the overkill approach

That's overkill. A lock washer on top of a normal washer and blue loctite will prevent any adequately bolt from removing itself during normal operations. We've been doing this at the shop for years and never had anybody coming back before because the screws backed out .
