SOLVED dropping out of the middle sprocket

my GF's mountain bike is dropping out of the middle sprocket down to the small. it is particularly bad under stress (going uphill)
the front derailleur is centered and not even touching the chain.
I changed the rather worn chain. same problem.
the middle sprocket seems very worn. could this be the problem?

Never Mind. sorted it.
hoirribly worn middle chainring replaced, problem solved.
