Insulated water bottles, really? I assume you mean for summer. How long do they keep your drink cool? I tried one, and it barely made a difference. There is a lot of ambient heat bearing on a little bottle in the big outdoors.

Used mine this morning on our ride and we are looking at a 96 degree heat index today.
Very hot and humid today in Minnesota. Nice breeze though.

The owner gave me two sets and I won't use other bottles.
They work great.
Even used them in the desert this summer.
I was glad I had them.

lined housing.
Its the one modern thing all my bikes share, so its the only modern thing I apparently cant live without.
Lined shift housing, with stainless cables, has allowed me to run bar ends under the tape and out at the stem without any sort of resistance or difficulty of use.
Lined brake housing has allowed me to keep original levers and still brake with the confidence that the lever will fully return and not get stuck.

SPDs, lycra, lined water bottles, suspension, brifters- its all convenience.
The topic isn't what you literally cant live without having- it means what modern convenience would you not want to give up over other things.

Clipless pedals.
Dual pivot brakes and decent pads. At (well) over 200lbs, being able to stop when I want to is huge.

Wet wool/chamois hand laundered in the sink after every ride left to eventually line dry only to be rubbed with the chamois potion of choice on the cob rough chamois compared to toss in the washer and hang dry overnight? No question: Synthetic.
Sagging water logged wool shorts in a long wet race or laden w/ mud in a 'cross race? Not fond memories.
So, do you still wear wool & leather chamois shorts?

Modern Campy shifters. Rode/raced downtubes for years, but glad to be rid of them. I really like being able to shift while climbing out of the saddle, since it's all short hills here.

Definitely LED dynamo lights. Without them, there's a pretty good chance I'd no longer be living.
Until recently I was living without a Hebie Chainglider, but now I don't think I could go back. Pants want to be free.

Modern chains and clipless pedals. I don't ride without these!
I also especially like a few of the modern saddles, and modern tires are great.

Okay, my first thought was my helmet, which may in-fact be quite literal in regards to the original question! But yeah, my crankbrothers pedals for sure.

@nlerner 9-speed Shimano with 11-speed Ergos works too. I have 2014 Athenas and it works with an Exage 500 rear on a 9-speed Shimano cassette; amazingly the Powershift works ok with the SRAM X5 front derailleur too.
