How do you install these types of toe clips?

I know this may be a total newbie question and I may be flamed hard for this, but I'm a bit confused as to how to install this certain type of toe clip:



It looks totally different from the installation of these types:

Those on top are for Campagnolo Aero Pedals. The others are traditional.

Also similar to the aero pedals from some other brands as well. My Peugeot has pedals (mailliard if memory serves) with that bolt pattern.

I once rode about 30 miles on aixaix's lovely bicycle which had that "triangular" style of pedal. I can't explain why, but it gave me excruciating pain. Someone I know said he experienced the same thing. That's all I can say.

I have the Dura Ace 7400 pedals on my bike currently. I have never had a problem with foot pain, with both sneakers and cycling shoes. I dunno, they work for my foot where as campy quills and track style pedals hurt my feet.
