Dia Compe brake failure!
This little feller just opened up and let go of my cable. Nobody got hurt but it was an eye opener.

I've had that happen, too - just after changing cables. Now I always inspect the anchor at the lever end from underneath and ride a few miles on a bike path after changing cables. So far, so good.

I just changed that cable. I bet I didn't get the end of it seated right in the ferrule or something like that. I'll have to look really close next time I do one of these.
