Dumb SIS question - How do you use the SIS Thumbshifter?

I have one of these:

on my recently found Miele mountain bike. The shifter moves the cable back and forth and clicks away as it does it, but how do you move from indexed to friction? The little knob on the shifter seems stuck in position, even though there is a little logo on the shift body that says F in one direction and SIS in the other, which I assume means use the little lever to select?

Anyway, help?

Also, the frame braze on to derailleur housing at the back was completely messed up - do I need that really stiff style of housing or will regular derailleur cable suffice?
