Hybrid road bike with upright geometry

Does anyone have any recommendations for a road bike that is sturdy, as upright as possible (dutch-like geometry), and comes with disc or hub brakes? Rack mounting points would be nice too.

Giant makes some hybrids with disc brakes. Not sure if they're upright enough.
