weird tube leak

Quick question: What would cause a leak 2 inches from the valve. There was a little hole about 1/3 inch in diameter. I don't quite get how the inside facing part of the tube would get a hole. No problems with the rim. Could it be a fatigue in the material.

Check rim strip, also any exposed nipples?

If it was a pinch flat, there would usually be two holes, but I have had ones with just one hole, or a hole so tiny It barely leaked.

Once you've checked and rechecked for all possible causes you have to consider the conclusion that some tubes are just defective. I very quickly went through a series of specialized branded tubes a few years ago that we're simply just a bad batch. My LBS made it right on the tubes but it was a very frustrating stretch.
