Best accessory for my commuter has been the AXIOM Columbus trunk bag. It holds the bike pump on the side, spare tube and tools in the back, a flasher light on a rear belt. Plenty of room inside and firm constuction with good stitching and reflective strips. It expands up (angled) and expands with fold down panniers. Didn't cost all that much and has proven to be a very good commuter trunk bag.

Hmm My coolest accessory?
hmmm. my bar end shifters shouldn't count, thats a modification, not an accessory.
I would say the Topeak Road Morph. Saved my ass, and made it look easy.

Not too cool, but useful are my panniers... little things now come on the ride with me, and I always have a place to toss things I get along the way.
I love having at least one bag with me... for the camera or whatever.
When I ride my other bikes, I miss the convenience of the bag.

I like my DIY LED lights, even though the system will probably never, ever actually be finished (there's always an upgrade). I get happy every time i turn them on.

This thread is awesome, I had never heard of a few things listed here. For instance the Crank Bros. Speedlever. I now have a few on order.
Keep 'em coming guys.

Back in the early 80s I had a little dealio called a Flick Stand. It had a small stiff loop of metal wire that mounted on the down tube - about where the front wheel comes closest to the down tube. When you wanted to lean your bike against something you moved the wire down until it touched the front tire, then you wheeled the bike backward about 3 inches so the wire locked in place. This then 1) kept the front wheel from turning and 2) kept the front tire pointing straight forward - locking the handlebars. When locked you could lean your bike against ANYTHING, no matter how small, and it was rock solid. It would not fall over.

Marathon Plus tires.
I am lazy. I don't like changing tires. So tires that don't get flats are awesome in my book.

Ok, now how many people have the same gadgets they had when this thread first died?
No, really, I'm curious.
I'm guessing the wait is over.

A marine horn which I keep strapped to the handlebars. It makes my presence known to the point that it scares dogs away to making truckers come to a halt in the middle of the street.
Two Cat-Eye blinkies on the handlebars. I also have three Planet Bike Superflash safety lights mounted on the back. Along with keeping me visible to motorists, they're great when riding in parades. I always use them when riding.
I also carry a whistle attached to the zipper of my jerseys on every ride in case the marine horn gives out.

My triathlon clip ons, the other day I rode 30km/h for about 7, maybe 8 km's. I was driving in the slipstream of a motorized scooter. Average speed blasted up.
