With both my trailer and regular trail-a-bike, I always use a flag. Flashing lights are good, too.

Flags always, I am actually thinking of replacing that tiny red little flag with a DIY larger one. Yesterday I saw such, big yellow triangular banner (more than half meter in the widest section), it attracts attention way better, too much visual noise on the busy streets to recognize that small little thing.
