Looks like the Salsa Down Under Rack from Universal Cycles:

Edit: I guess Salsa is the name of the manufacturer, and Univesal Cycles is an online seller.

He's right it's made by Salsa, which is a QBP brand so most any bike shop should be able to order it.
I just got one for my Trucker and I really like it. Keeps the bags low and is very lightweight.

A variation of a low rider rack, as originated by Blackburn in the '80's.
