I don't ride clipless pedals , but back in the day when I was riding a road bike with toe clips I strapped in tight for the first time and joined the club soon after.
I was riding with a couple of friends when it started to mist. One of the guys was riding around a curve and the bike went out from under him because of the wet pavement. He bounced his head off of the pavement, but that was the first year he had started to wear a helmet, so he ended up being fine. I turned around to see if he was okay but when I pulled up next to him I forgot to loosen my toe strap. I came to a complete stop then fell over onto my left side. So, there we both were, laying in the street getting rained on while the other guy was getting a good laugh at our expense.

Witnesses: I rolled over backwards as the momentum of the tombay sent me head-over-heals and I looked around to see if any-one saw; and there was only one person laughing... my wife. So, only one witness.
Embarassment level: I guess on a scale of 1 to 10 with 1 being the least embarrasing and 10 being the most embarrasing I would rate this event an 8 because, well, it was embarrasing.
Injury: Hip is a little sore but pride is hurt the most.

You've got my vote in fact, I'm awarding bonus points because it was witnessed by your wife.
Other witnesses will laugh once in your face, maybe tell a bunch of their friends whom you don't know, and be done with it. Your wife will find ways to bring it up for a very long time to come.

Description: The wife and I were on a ride and we were late leaving the turnaround break-point at the 10 mile mark. (It was a 20 mile ride), the rest of the group was speeding away and we had to cross a freeway; not much traffic but there were a couple of cars approaching so we had to yield the right of way. I unclipped my left food and of course my center of gravity pulled me to the right and down I went; my right hip absorbing the blow.

The presence of your beautiful bride negrates the necessity of the young nurses to be involved with your Tombay. Now if you think you've heard the last of this from your beautiful bride think again.Don't be surprised to have her hand you hip protection before your next ride.

I'm currently sporting a sizable bruise from a fall a couple days ago. It happens, fortunately not often. Congrats.

I've learned the canoeing equivalent of a tombay.
One foot on the dock, one foot in the canoe. Do the splits.
On my first one, January 1, I went in, over my head, filled the canoe with water and dumped my son.
On my second one, February 11, I was able to make it back to the dock before the canoe filled. Only my ankles got wet. Maybe that doesn't count as a tombay. Just a close call.
No pictures. #1 witnessed by my son, and an outrigger canoe foursome who helped me bail out. #2 witnessed by my girlfriend who remained calmly seated in the bow of the canoe.
Embarassment? Yes.

I saw a nice initiation yesterday at the start of the Cascade Bicycle Club Chilly Hilly ride.
The ride involves taking a car ferry from Seattle Washington to Bainbridge Island and then circumnavigating island. One of the riders managed to do the deed before he even got off the ferry, surrounded by literally hundreds of fellow cyclists.

Took me a bit of research to find out what this was all about. Around here, we call that move an "Artie Johnson" with a tip of the hat to the old Laugh-in Show. Yeah, I did that last week on the way to work. Stopped at a traffie light that takes forever to change, and was find, but clipped in and fell over trying to take off when the light finally changed (too high a gear). I got up, walked to the curb, and pushed the button on the cross-walk, walked across the highway and then got on and rode away from the other side.
