Ok, so I've always had this at various degrees since getting into this seriously in 2010. I've always used the same Shimano shoes with look pedals and cleats.

I rotate through 4 or 5 main bikes and the symptoms happen with all of them. In longer rides (usually more than say 25 miles) the outside of my forefoot gets really painful. It's the only real fit issue I have. I know I have a habit of walking on the outside of my foot and rolling it. It's the country boy who likes to move silently in the woods part of me I suspect.

So during my fit I had cleat wedges installed to take care of a difference in leg length or whatever. Would Valgus wedges inside the shoe take care of this this problem?

I should not that in the winter I wear the same shoes but tape up the vents, remove the insoles, and wear thick wool socks. Same pain...

Looking for a solution I can try myself before I try to find another good fitter. That seems to be hit and miss...

I would suspect your feet swell with use causing a tight fit. If possible with your shoes, loosen the fit in the forefoot area. I have laces on my bike shoes but leave the toe area fairly loose for several eyelets, then tie a knot and tie the other eyelets at a normal tension. With velcro straps I leave the lowest strap loose then normal tightness on the upper straps. Hope that helps.

If it happens with multiple pairs of shoes that feel good at first, it could be a 'tailor's bunion'.

Well, with more thought and consideration about this I realized it's the whole forefoot, just worse on the outside. Yesterday I put some metatarsal pads temporarily in my shoes. The right was fine but the pad slipped on the left.

Tomorrow I plan on another century so we'll see what happens.

The pads did the trick. $6 solution to something I've been putting up with for years.

I have a similar problem with my left outer forefoot. For the first 5 miles or so it hurts, then it stops. This just started happening recently. Doesn't seem to be the shoes themselves, so I'm thinking I might need new insoles.
