When Salsa goes for a road ride:
spare tube & tools to fix a flat (w/ big air cannister)
water
power/conex link & chain tool
cell phone

Mountain bike ride:
same as above
some sort of powerbar
all-purpose tool
bits of duct tape for emergencies (2 wraps around the head tube will make a decent bandage)

When going on a ride for fun, I bring a spare tube, tire levers, a multitool, pinking shears, a chain tool, water, headlight & blinkies, a top hat, some change, a knife, a pump, and sometimes my wallet and a cell phone.

When commuting, add to that panniers, a rack trunk, an AK-47, handlebar bag, chain lube, a change of boots, a belt, a Faberge egg, three pterodactyls, reading material, a sweater (I need to take that out, sweater weather is done for the year...), and change the "sometimes a wallet and cell phone" to always the wallet and two cell phones.

I've actually ditched the multitool in favor of the right sized allen wrenches (2) and a leatherman. It seems that every flat I get I gotta pull something out of the tire. Once it was a dry wall screw...I used the phillips head screw driver.

When I go riding:

Tube
Tire levers
Allen wrench set
Crescent wrench
Toilet paper
Knife
Water(sometimes water insert)
Cell phone
Migraine pills
Codeine aspirin- just in case I'm in some kind of emotional or physical pain
Wallet +money
Camera
Patch kit

When I go riding with Siu-
Water
Camera
Wallet
Cell phone

I dump everything else out before I go, she can't stand clicking and is a pretty good mechanic, so I just rely on her.
