Where can I get a quality customized cycling shirt made?
This year I'm going to be riding in the Massachusett's Pan Mass Challenge. The first day you have to wear the PMC bike shirt, but you are free to wear a shirt of your choosing on the second day. I have the potential to get a fairly sizable donation, if I wear the logo of place on the second day. I have no problem with this, but I have no idea where I could get a good shirt made. I'd be looking for a pullover quarter zip short sleeve shirt made out of quality durable moisture wicking fabric.
If anyone knows of a place, I'd appreciate the information, number, web site, etc'.

I'm assuming that you only want 1.
Look for a custom T-shirt place. Many of them can photo-copy a color logo of your choice onto a plain jersey.

There was a thread on this a while ago, but I couldn't find it. There are a few places online that do custom jerseys for teams. If you ask, they'll also do individual jerseys, but will charge quite a bit for it.
I looked into getting sponsors, which can cut the cost dramatically.
For an underarmour-style shirt, it looks like the customink link Ccrew posted is a good bet.

Best way to do it quickly in my opinion is get some iron on transfer paper from any local stationary computer store. I have even found it at Big Lots. Run up the Logo on your computer and print it on the paper. It works best on a light colored Jersey. Remember to flip the image on the computer. You can iron it on and have it in less than an hour. It will not last as long as a custom professional job but it will only cost a few bucks at the most.
