Installing Cleats on Shoes

Just bought a new pair of shoes, need to install cleats on shoes. The packet of cleats did not come with instructions. Not sure about one part (see photo).
Top piece, I think is to be discarded. I know what to do with all the parts, but the bottom piece. Is this an extra part?

Is the bottom piece threaded? Looks like it may be for the inside of the shoe if needed, but hard to tell due to focus. My shoes already had plates in the footbeds so that piece would be discarded in my case.

Yes, it is threaded. Makes sense, yes my shoes do already have the threads in them.

What shoes are you using?

If they are SPD compatible mountain bike shoes, this might help.
