It's called simply enough a Top Cap!

There is a company that has a top cap that allows you to install a bottle cap as a decorative cap. Can't remember the name.

Marzocchi has a pretty cool one!

I like my Chris King.

I want a top cap in the form of the Staples (office supply) "Easy" button. Does anyone make one?

It all depends on your cable routing and frame geometry.

A top swing derailleur has the seat tube clamp placed low and the derailleur sits on top of it. A lot of full suspension bikes with an interupted seat tube use this type. A bottom swing has the clamp up high and the derailleur kinda hands down below the clamp. A standard triangle frame utilizes this type.

If your cable routing goes down your downtube, around your bottom bracket and then up, you need a bottom pull. If your routing goes along the top tube and down the seattube, you need a top pull.
