TDF Crashes

So I was wondering if there has ever been an incident when a rider(s) crashed and a car or motorcyle following couldn't stop and hit the cyclist(s).

It gets really crazy watching some of these stages with narrow roads, cyclists going 30mph, sharp curves and you have motorcycles weaving in and out and cars weaving in an out. And you know that several drivers are the DS and they are driving and talking on the radios at the sametime. It seems to me that this is all an accident waiting to happen, so this got me thinking.
