Im using the Megas. I tried on several shoes from Shimano and Specialized and didnt really run into anything any wider. I think Shimano has a wide width shoe which I was never able to find anywhere in stock, they might be wider or as wide.

I have some pretty wide feet (14E) I just got a pair of Nike Altea II's in a 49 (14.5 on the box) they seem to give me suitable room for my duck feet. I don't really see the need for specifically wide footwear most of the time. Go figure

Specialized shoes are fairly wide. More room in the toe box than the wide Shimanos. I haven't tried the Sidi Megas.

I came to this board with the same question a few months ago when I got back into road biking, and went with the sidi mega's. They are roomy in the toe box, but their actual width in the widest part of the foot is not much wider than a regular road shoe. My feet started hurting on the outside of my foot 30 minutes into a ride.
I then tried the specialized comp carbon road, because they are a wide platform, but I had the same issues. Both shoes seem like great shoes, the sidis are a solid shoe and would probably provide many years of good use as long as your foot isn't too wide. I love the way the specialized bootie gripped my feet and made them feel secure, but the platform was just too narrow in the forefoot.
I still own the Sidis, but returned the Specialized to my LBS after 2 rides. They ordered me some Shimano SH-R215's in the wide version. These are the best ones yet. The forefoot width is the widest of all of them, but if youre were to try them all on in one sitting you might think they were the narrowest because the toe box is the tightest. They have some plastic bits running down the side which theoretically provide support (which I'm sure they do). However, after riding a few hundred clicks with them, they are by far the widest shoe, although of the three they feel the tightest in the toe box. I can live with a tight toe box, but if you truely have wide feet like I do it's not pleasant to always feel that pain on the outside of your foot after 30km. I'd rather have cramped toes.
If they fit, I think all of the shoes are good shoes. If you have fat toes (?) the Sidis or Specialized will work. But after measuring all 3, the Shimano's are the widest. If anyone is looking for some like new Sidi's in a 44 let me know...

Ride it like you stole it
I also use the Sidi Megas. What I understand is that the Megas are like a EE-EEE, if you need it a little bit wider just go up a 1/2 size. Although half sizes are very hard to find in stock. Theory is if a 45 is just a hair narrow the 45.5 will be the same size just a little wider. Don't know where I read that but I do remember seeing it in print somewhere.

I went with Lake mtb shoes, rather than road shoes. They make wide versions.

My heel is narrow to normal while the front of my foot is wide. I use Pearl Izumis and they work absolutely GREAT. I have a pair of Specialized that seem pretty good and the Sidis Megas made my feet hurt after about 50 miles before I finally broke them in and now they seem to work OK.
Lake have very low sides and the heel is wider. The end result for me was that the shoe flops up and down if I try to pedal wearing it. I had the same problem with the Northwave.
I wear 11 1/2 - 12 shoes. If the shoe is on the smaller last I couldn't judge these shoes accurately.

I've got fairly wide feet (New Balance 11.5EEE or 12EE for comparison). I've used Lakes in their wide width and they fit well but I had other issues (hotspots).
I currently wear Specialized and find they fit my wide in the toebox but otherwise normal feet pretty well.
I've tried on Nike Ventoux IIs in wide and couldn't get close. (A great disappointment as I could get them at the shop price).
Haven't tried the Sidi Megas yet but did buy, real cheap off eBay, a pair of their Romanian made Scarpe Dynamics and found they have a decent fit. Apparently these made for Sidi shoes have a different fit.
The Specializeds are a better shoe and my fave but I'll keep my eye on these Sidi Scarpes.

I finally abandoned my Sidis. They were plenty wide enough, but the arch was just too high, and the custom insoles were never as wide as the Sidis, and that made the shoes plenty uncomfy.
Just switched to Specialized BG. Not quite as wide as the Sidis, so I hope they stretch a little with use.

What do people think makes more difference for room in the toebox:
-wide disignation (eg Sidi Mega) OR
-go up a 1/2 or 1 size?
Am thinking winter time, more room for socks/heaters/extra wiggle room to increase circulation. Currently wear Sidi Mega 42 in summer which are snug w/ thin sock. Winter am wearing Mega 43 w/ room for smartwool thick socks, but would not call "roomy". Toebox more important than width at ball of foot in this situation IMO.

Pearl Izumi's seem to be a lot wider- worth checking out. I think their sizes run a little big too.

Decided to blow my budget for the whole month on another pair of Sidi Megas. 44's. Don't want to scrimp on the feet. Comfort too important. $179 from Biketiresdirect.com.
Now will have the following size shoe for the given conditions:
42: short rides in warm weather only
43: longer rides or cool weather
44: cold weather only

you can try finding a Carnac wide width shoe from last year, because they stopped making them, but i think it will be about the same width as the mega you're using now...

Tried on a Specialized model (Comps I believe) in LBS. They do also seem to run on the wide side; they are wide and roomy in toebox, but a little narrower at the ball of the foot than the Sidi Megas.

Interesting to hear about the specialized shoes. Have always want to try those, now if I can just find them somewhere. Someone posted elsewhere that the Sidi wide or Megas were only 5mm wider. I have pretty wide feet and my Mega 5 Lorica's feel wider but no real wide.

I'm sure, but they are very expensive and quite frankly about as nice looking as the Pontiac Aztec.

The Aerators are wide?? What size are you feet. Have had them on in the store and they didn't give me any extra room in my normal size.
