I've been using Q-rings on both my road bikes and on my TT bike for about a year. I use a PowerTap and I really can't see any difference over a short time period, but I do have a slightly higher average over longer rides (my hip flexors usually tires, but this doesn't happen to the same extent with the Q-rings). I've had knee issues in the past and it seems the Q rings do help with that.
I definately do climb better with the q-rings (I tend to sit most of the time climbing, when I stand, I think I would prefer a standard round ring as the q-rings get a bit "bouncy"). A negative is that the front shifting is not as crips as it is with a round ring... you basically will have to wait for the ring to get to a position where the chain moves up (if you think about it, it's only fraction of seconds though, but noticeable). No real issue, just a different feeling

Have them, love them, no negatives that I've found. Very easy to get use to, I miss them when I ride my older bike. Even at 3% improvement, that is around 10 watts. I hope no one else in my group buys a set, nice to have a little advantage.

I have them and I like them a lot. I have a stretched Post Tibular Tendon and I just started wearing orthotics. The Rotor Agilis Q-Rings work really well for me and I like them a lot. They took me about an hour to get acclimated to them. They definitely help my injured foot work more effectively and painlessly. It took two trips to my bike mechanic to get them dialed in with my Dura Ace 7800 front dérailleur and Dura Ace 7900 chain. I highly recommend them.

To add to my OP. I have since purchased them and am entirely happy with them. I didn't suffer from poor pedaling mechanics or injury, rather, just wanted to see the difference if any. I notice that over distance I am able to hold a bit more power. Not that I am producing more, but am just able to sustain it a bit better. I can see how these could be a godsend to people with a tendency to mash their pedals. I recommend them to anyone interested.
+1 on it taking a few tweaks to get the front shifting dialed, but it is possible and shifts plenty smooth now.
